The Cardiac Rehabilitation Program is a four-phase outpatient program of supervised exercise and education, specially designed to help patients who have suffered a recent cardiac event (or are at high risk of a cardiac event) to return to and maintain as high a quality of life as possible. A cardiac event describes several types of serious cardiac conditions, including heart attack, angina, heart bypass surgery, angioplasty, valve repair, heart failure or heart transplant.
The program is offered at state-of-the-art facilities at Day Kimball Hospital. It is staffed by a team of registered nurses and exercise physiologists who are educated and experienced in assisting cardiac patients in recovering from and preventing heart disease. The program meets national standards of excellence in cardiopulmonary rehabilitation patient care.
Phase 1 provides inpatient consultation to patients who are hospitalized due to a cardiac event.
Phase 2 provides telemetry monitored exercise sessions three times per week for patients who have had a recent cardiac event. This phase is covered by most insurance plans and lasts up to three months (36 sessions).
Phase 3 provides exercise and education for the patient who has not had a recent cardiac event but wishes to reduce his or her risk of developing heart disease. The duration of this phase is one to two months. As phase 3 is not covered by most insurance plans, it is a self-pay phase costing $103 per month, billed to the patient directly.
Phase 4, also known as the “maintenance phase,” is for people who have completed either phase 2 or 3 and wish to continue with their exercise under the supervision of the cardiac rehab staff. Individuals who have graduated from other cardiac rehabilitation programs are also welcome. Phase 4 is not covered by most insurance plans and is therefore a self-pay phase costing $46.15 per month, billed to the patient directly.
A referral from a primary care physician or cardiologist is required to enter the program. Your first visit consists of an interview and thorough assessment of your medical history. You will be asked to fill out informational forms to help us plan for your educational needs. There will be a brief exercise session to establish your exercise prescription.
Open to cardiopulmonary rehab patients and the general public.
Free heart disease education classes are held every Wednesday in the Cardiopulmonary Rehabilitation Center at Day Kimball Hospital from 10:30 to 11:30 a.m. The classes help to build awareness and understanding of heart disease, and to help prevent future cardiac events through lifestyle modification. The classes are held on a “drop-in” basis; no pre-registration is necessary.
Billing for participation in the Pulmonary Rehabilitaiton Program will come from Day Kimball Hospital.
